He was born in Calgary, Alberta on June 25, 1971 and was given the first name Lee at birth and was adopted at 7 weeks old. He was born premature at Calgary hospital. He has brown hair and brown eyes. His mother was Ukranian and father was Norwegian and his father had a twin sister. He has a younger sister a few years later when his birth parents got married. It was a closed adoption.
Please provide any information if possible on family history of health that is vital.

The adoption records state that our birth mother was Joan Dorothy Simpson born Jan 15, 1932 and was from the Edmonton, Alberta area. Our birth father was 23 when we were born but only said he had two sisters, no name was given. Not sure if our mother’s name is correct as it states that she did not want her family to know of her condition. Ancestry DNA shows Johnston and Martin surnames as second cousins or closer.

Around 2003 her boyfriend contacted my father in Yellowknife NT. A voice message was left on my fathers answering machine. A phone number was left but only area code the rest of the number was cut off. The area code was around Kitchener Waterloo In Ontario. The boyfriend left her name and birthdate and where she was born and said she was looking for her birth mother Cindy Jeske.
