My birth mother was 18 years old when she gave birth and experiencing her first pregnancy
(EDO 10/9/70) when she requested counseling and permanency planning. She had learned of the
pregnancy prior to relocating to Florida to stay with a family friend and had begun counseling with the local Catholic Charities there. She was referred to that agency by her priest. The adoption worker then made a referral for collaborative services on 9/3/70 in order to facilitate a timlier adoptive placement upon the child’s birth. My birth mother was Catholic, Caucasian and of Irish, German, English, Dutch and French nationalities. She was described as 5’ 4" tall, 120 lbs with a medium build, curly dark brown hair, brown eyes with long lashes, an oval face and an olive complexion. She had a pleasant, even tempered personality, and reportedly got along well with others.
Your birth father’s father was an approximately 45 years old, African-American. He was described
as 6’ tall, approximately 160 lbs with a medium build and brown hair and eyes. - His mother was approximately 40 year old, (Caucasian or of fair complexion) woman. She was described as 5’9" tall, 150 lbs with a large
build, brown hair and eyes and an olive complexion. He had an 18 year old brother who was
described as 5’ 6" tall, 140 pounds a 17 year old sister who was 5’4" tall, 120 lbs and an ll year old sister who was approximately 5’ tall. All of his siblings reportedly had medium builds, brown hair and eyes and were light complexioned.
My birth mother had attended Catholic schools throughout her childhood and was a high school graduate, where
she had earned primarily average grades. She had taken college preparatory classes and had two
years of typing. She liked to draw and was artistically inclined. She had been extended an Art college scholarship upon graduation, but was not able to accept it. She did have an interest in sculpting too. She hoped to attend night school in the near future and very much wanted to further her career goals. She had worked while in high school as a cashier, then after graduation was a bank teller, had receptionist employment and later worked for a loan company. Before relocating she had worked as an expediter doing clerical and inventory control work in manufacturing. She enjoyed reading in her spare time.
My birth father was 21 years old and of bi-racial heritage. He was described as 6’ tall, l35# with a thin, slender build, brown hair and eyes and a light brown complexion. He was a high school graduate of average intelligence and had also completed computer programming at a technical college following graduation. He was employed in the computer programming field.
My birth mother’s father was 44 years old when she made her adoption plan with the agency. He
was described as 6’ 4" tall, approximately 160# with a large, muscular build, blonde hair turning
gray, blue eyes and a light complexion. He was of Dutch and German descent and had converted
to Catholicism. He was a high school graduate who worked for the government as an office chief.
He was reportedly very intelligent, and enjoyed golf in his spare time. Her mother was 40 years
old. She was described as 5’4" tall, 115 lbs with a medium build, brown hair and eyes and an olive
complexion. She was of English and French nationalities. She had worked for a large newspaper publisher after graduating from high school. My birth mother had a 21 year old brother who was described as 5’ 8" tall, 135# with a medium build, curly blonde hair, blue eyes and a light complexion. He was a high school graduate. He had a girlfriend, and was working in her family’s service station. He wanted to learn the mechanics trade and also wanted to take college courses.
My birth mothers sister was 10 years old. She was described as approximately 4’ 6" tall with a thin build,
blonde hair, brown eyes and a light complexion. She was an average student in the fifth grade.
Her father was described as having "a bad heart" but he and his family were reported to have no hereditary diseases or other health conditions.

My birth mother was reportedly in "good general health." She had the usual childhood diseases including measles, mumps and chicken pox. She had recurrent kidney infections for two months during the winter of 1969 and indicated that she had a slight spinal birth defect which caused her back to give out at times. She also identified that she had a pinched nerve in her left leg. She reported that none of these ailments were serious, and she did not experience any significant health issues as a result. She also disclosed that she was allergic to bee stings, and would require an allergic injection should she be stung. She denied all forms of substance abuse and stated that
there were no other known hereditary conditions in her family.
My birth mother had started prenatal care prior to delivery, but did not have her first out of state visit until 9/8170. Her serology completed on 9/9170 was non-reactive, and the vaginal smear on 9/8/70 was negative. Her throat on 9/8/70 was normal. She had a diagnosis of abnormal flora in her vaginal discharge on 10/9170, but she was treated orally with Flagyl and this condition was fully resolved. A culture taken 10/15/70 showed "normal flora." She had a chest X-rayon 10115/70, which was within normal limits. Your birth mother had been living with her parents in the Midwest, and reported a good
relationship with her family, and appeared especially close with her older brother. In the year prior to your birth she had shared an apartment for a few months with two of her girlfriends. She had known your birth father for over a year, but they had stopped seeing one another March 1970. She chose adoption and appeared to actively plan for your well-being during her last trimester. She did want her confidentiality, and it was important to her to take ownership of her decisions and outcomes. She received counseling from the Catholic Charities agency, and was fully responsible for her financial obligations. She declined further counseling following placement, as she was comfortable with the services extended. Her case was closed 1131171. The record does not reflect any post-finalization contact by your birth mother or any other birth family
members. My adoptive parents had started the adoption process in June 1969 while living in
Santa Ana, California. They subsequently moved to Newark, Ohio and very much wanted to
continue with the home study assessment. They applied to adopt through this agency
approximately 4/21/70, and the home study was initiated the following month. Following my
birth, I was returned to Ohio by adoption staff with this agency. I was placed in foster care on 10/20/1970 and remained there until my adoptive placement was made on 10/30/1970. I was baptized on 11/15/19
70 at Church of the Nativity in Utica, Ohio. My adoption was finalized on 8/26/71 in Licking County Probate Court.
