My child was born Nathan Thomas Hill on 12/03/1996‚ at Alta View Hospital in Sandy Utah‚ delivered by Dr. Kenneth Larson. He was adopted by a couple with the first names Nick and Anna when he was 2 months old‚ through LDS social services. Our case worker was Connie Mason‚ and through Connie‚ I passed and received correspondence until Nathan was 14 1/2 years old. I do‚ also have a copy of his SS card and his birth certificate‚ if needed.
