I was born at Utah Valley Regional Medical Center in Provo, UT. My birth mother was from Chicago, IL, her father’s name is Peter Lundquist, her name is Dawn Hoffman. She has at least one sister. She previously gave a son up for adoption who was born 15 months prior to my birth. My birth father’s name is Richard Ottly or Ottley or Otly or Otley. Both of my birth parents were students at BYU in Provo, UT, they were not married at the time of my birth and went on to marry different people. My birth mother lived in Issaquah, WA at some point in the late 80’s/early 90’s. I was told my birth father was some percentage Cherokee and at the time he was in school at BYU enjoyed fencing and has blue eyes. Both birth parents are Caucasian.
