The adoption agency misspelled my last name on the papers and I had to initial the correction each time they typed Fritzgerald rather than Fitzgerald. I have recently discovered from the State of Oregon the adoption was not filed in Oregon. I chose the State of Utah since LDS Family Services is headquartered there.
