My birth name was Sally Johnson. My birth mother’s name was Eleanor Jane Johnson. I was the youngest of four children. I have no information concerning my birth father. The last known address for my birth mother was a P.O. Box located in Bainbridge, Ohio. The Dr. who deliverd me was Dr. McKell and I was born at the Chillicothe Hospital on March 26th, 1963. I was adopted on June 18th, 1963. My adoption was finalized in 1964. My records have been sealed until congress recently past a law permitting sealed adoption records to be opened upon request of the adopted child and permissable with the birth parents. I am facing some health challenges and would like to obtain any medical histories that I could. I would also like to establish a relationship with each of my birth parents if possible. I also have three siblings, whom I have never met and would like to find each of them if possible.
