To whom it may concern. I am in search of a way to order a copy of my adoption certificate/record. I was raised with my biological mother and learned that my father was not my biological when I was 14. At 17 I was diagnosed with cancer and discovered who my biological father is. Today I am 43 years old‚ have a relationship with both biological parents but more importantly would love a copy of the paperwork that states when my “stepfather” officially adopted me and gave me his name. He has since passed away and considering I was raised my entire life with him as my primary father figure‚ this record is the most important document bonding me with him. I am now married resulting in my name change so having something to reinforce my maiden name is crucial for my child to understand. I saw the original adoption paperwork years ago when I was a child‚ but we lost it to a flood. I was too young to figure out how to get a replacement‚ so I left it alone. Now with him gone I’d love nothing more than to get a copy of that. I don’t know how or who to contact so attempting this request with the address on the ny.gov site. I can have either living parent sign anything or confirm this‚ if needed. Further Information: My maiden name: Marjoriet Toro Matute (B. 5/20/1982 Jacobi Hospital Bronx NY) Biological Mother: Sandra Mary Toro (B. 3/8/54 Bronx‚ NY) Adopted Father: Absalon Matute (B. 12/11/1950 Guatemala) My parents were married 10/29/1982 New York Once they were married‚ I believe is when my name changed to “MATUTE” My original birth certificate states “ROBLES” as my mom and my last name. This was from my mom’s first marriage. (NOT MY BIOLOGICAL FATHER) Hope this helps without giving my social.
