I pray this finds you well. I was born and adopted in the state of NY. I write with the most anxious hope that you could please help me acquire any information regarding myself and my adoption. I am in dire need of both my original PRE adoption birth certificate‚ as well as any/all court documents ordering/authorizing my adoption and name change. I was born jan. 15‚ 1964 in the town of Barton in Tioga County‚ N. Y. to whom I am told is a single mother who handed me over to the Angel Guardian Home adoption agency. My birth first name was Christopher (from my birth mother and my adopted parents kept). My adopted parents THINK my birth last name was FERRY‚ but they are not sure. They are in their early 90s. I was temporarily handed over to someone to care for me. I was finally adopted at age 18 months by Aniceto John and Maureen Frances Serrone from Angel Guardian Home. The only documentation of any of this is a POST-adoption birth certificate certifying my name as Christopher Serrone‚ born 1-15-64 in the town of Bartlet in Tioga county NY‚ as shown by the duly registered original certificate of birth filed on Jan. 21 1964 in the local registration district ‚ and on Feb 11‚ 1964 in this department. It is signed by bureu of vital records Joseph H. Sterzinger‚ Director dated Dec. 12‚ 1966 in Albany NY under seal. It is given certificate no. 12687. That’s all I have. It’s original‚ but POST adoption. To understand my situation I also need to explain one thing. On Dec 7‚ 1979 at age 15 I was arrested and sent to adult prison. I served continually and didn’t get out until Dec. 19‚ 2024. And I was released from prison with lung cancer that spread to my brain requiring multiple massive surgeries. Thus I am on constant oxygen‚ use an electric wheelchair‚ and am formally designated disabled by social security. My only income is SSI of only $1025 per month. So all this beurocacy and technology is new and overwhelming to me. I need help. I’m struggling. Also‚ I am desperately seeking new housing under a government assistance program. Otherwise I’m soon to be homeless. I’ve been told I can’t get on waiting lists because my POST adoption birth certificate is not a "real" birth certificate‚ and I must go get my original birth certificate. But if I were to bresemt my original birth certificate‚ it would be under a different name and I would be denied anyway. Thus‚ in ADDITION to my original birth certificate‚ I’d need a copy of the original court order authorizing the adoption and name change from the old name to Christopher Serrone‚ just to prove who I am! Besides‚ it would be a wise thing to have such an order. I hoped you could PLEASE help me with what info you can provide. I also hoped you could tell me exactly what court authorized my adoption and with whom I must file a motion to unseal and obtain a copy of my own adoption. Finally‚ I hoped you could please refer me to a low cost or free agency that will file in the courts for me. Upon release from prison I was deemed disabled‚ and only get SSI.
