Had her at Copley memorial hospital in Aurora Illinois I think adoption parents names are: Tom and Linda At birth‚ I named her Elizabeth Chapman however‚ name was changed. My name: Lisa oliva dads name: Brian Chapman I was 15 years old had her November 3‚ 1993 They may have named her Kelly I do have one picture of her as a baby. I signed up at a place ( cant remember name) that I give my health records to so incase she needs them. Found paper so I went through: Cook county department of supportive services‚ Chicago il Date I adopted her out is 11/8/1993 I actually named her Elizabeth Nicole Chapman Had her at 6:37 pm
