My birth name was Mary Laura Salazar and I was born on June 29, 1973 at 4:47 a.m. in Chicago, Cook County IL. My birth Mother’s name on court documents is Laura Concepcion Salazar. My Birth Father was served by the guardian ad litem and didn’t respond. He was declared unfit and had not seen me since birth. My adoption decree was dated May 6, 1975. I’ve had some medical issues so I’ve decided to seek out my biological birth parents to see if I can obtain health history. I will also submit my information per the new IL laws to obtain all records they may have relative to my birth.
