I was born in Moline Public Hospital ‚ which is closed down now from my understanding‚ to a woman aged 19years by the name of Peggy Lee Swanson. I do not have an address or a date of birth for this individual. The address on the birth certificate is 220-11th Avenue‚ Moline‚ IL which I believe was the hospitals’ address. The name of the doctor who signed my original birth certificate was W.H. Otis‚ M.D. I was born in Rock Island County‚ Illinois. I was adopted through Bethany Homes-from what I was told. Other than that‚ I know nothing.
