University of CHGO Medical Center - Lying-In Hospital. Believe it was a "doctor-arranged adoption". Was intimated of this by a social worker in about 1974. I was 24 years old. I had been on a re-call list as "a child of the mother given DES while pregnant with this child eventually may contribute to clear cell cervical carcinoma". My Mom was extremely adamant that the hospital bring up my original medical record number. No such number existed!
