Birth name given was Diane Michelle and to my knowledge, only first name changed. I was adopted within the first week of birth, and my adoptive parents lived in Lake Cindy/Hampton, Georgia (Henry County) at time of adoption. I’m pretty certain my adoption was through the State’s Department of Family/Child Services, not private. I understand from my adoptive mother that I was born in South Georgia, even though my birth certificate lists Griffin-Spalding Hospital which was the local hospital to my parents. I also understand that my father was not listed on the birth/adoption documents.
