I was born in Tampa and was adopted out by the LDS Family Services adoption agency about 4 weeks after birth to the parents Clayton and Carole Chevrier. I’m not sure what hospital. I was born male named Jonathan Isaac on my birth certificate but it was signed by my adopted parents. I recently requested that name on the birth certificate records changed to Amberleigh Mara as i am transgender.
