William F. Smith 43‚ Birth PA Married 1948 Lola M. Ayers‚ Smith 25 lived on Prospect St. Jacksonville‚ Florida at the time of my birth. 01/29/51 maybe? I was given an affidavit stating that Pauline A. Pelletier was giving me to the Smiths Notary was Ralph Danson I was told that she had been staying at the Salvation Army and that she came to take care of my adoptive mother. Hospital (former) Duval Medical Center‚ changed to St. Lukes Hospital. Birth father from Cyprus I have 2 birth certificates‚ 1 with a clear number‚ and the second with a sticker number on top‚ birth certificate was hand written‚ and it was amended in 1957. The Last name is supposed to be "Smith"‚ but looks like it started with SA‚ and possibly written over a name. I went by the name Taylor until I was 15 and went to get a work permit. I didn’t know I was a Smith‚ or adopted. I’ve submitted my DNA to try and find the truth.
