The date on of the adoption on that form is April 11th, 1961. I have received immigration files which indicate my birth mother’s name was Jaclyn Chatard. I was born in December of 1972 on the 24th in Kenitra, Morocco to an underaged Moroccan mother of approximately 15 years of age and a French Father. My birth mother and I shared the same name, but my fathers name was omitted from my birth certificate. I was told by my adoptive mother that my father’s father was the French ambassador to Morocco at that time however I have no proof of this.
